You only have to firestop when penetrating a fire/smoke rated assembly, i.e. wall, ceiling, etc.
We have 1 hr ceilings throughout the whole job, (2) 4 hr masonry fire walls, and lots of other 1 hr fire/smoke walls.
Of course it's not a bad idea to do it even where it is not required by code.
As for how many you can run, it's been covered. I think, a good rule of thumb would be not to fill the conduit more than 1/2 - 3/4 full, but I'm not an electrician.

Don't forget many phone systems are moving to VoIP so you should plan on having an IP connection for each stand-alone phone and one for each computer with a phone next to it.

Why not? If you only have a limited amount of wire, it only makes sense to use what you have. On short runs, you won't have enough crosstalk to effect data or voice, even with analog phones. Splitting up a cable and using a few unused pairs is a lot more cost efficient than ripping out and pulling more copper. Why not use the superfluous pairs if you have good performance?

I have done it and our engineers have tested as well, there is problem with it. We have several thousand houses in our fiber neighborhoods and ALL have their internet run in on a phone line with split pairs. Ethernet cables for internet only are only useing two pairs(green and Orange) in house with dialtone the blue handles the DT and the green and orange pairs handle the data, with the brown as a spare.
