It happens, I've turned loose once because it was either that or go swimming, got the fish and gaff back though.
I did stick a 100lb wahoo once that had run up between the motors, dead center, running about 12 knots and that SOB turned sideways and down. I held on so hard i thought my hands were broke afterwards.

First off, the girl with the gaff shouldn't' be on the boat.
Second, the gaff should be positioned behind the wire.
Third, the caption "green tuna" was written by an idiot. Assume every tuna is going to be rowdy on the wire. That weak faggot sank the gaff and held on. You have to pull the gaff and keep it moving up and over the gunnel. That fish was mortally injured by a crowd of stupid fucking assholes.
I have personally never lost a gaff to a fish. I have gaffed thousands of tuna and quite a few 100+ pound bigeyes, usually back-to-back from multiple hook-ups. EDIT - I should add here, I wired and gaffed those bigeyes. Was a time when charter boats only had one deckhand. They were always heavy and wound-up AF.
