Has anybody ever fallen out of a tree stand, hanging in the air? How did you get back into your stand? Or how did you get down?

Nope,
But I'm convinced that a correct setup would barely allow you to 'fall'.
Maybe a foot or so total vertical.
After asking here, I got a rock climbing belt and it is pretty awesome.
I've fooled around here at the house and it seems fairly easy to get back oriented and in a stand.

I agree with Sheep. Set your lanyard and tether up as a restraint. Regardless of how careful you are or how well you maintain your gear, things can break or slip, and a fall in a harness is still likely to hurt you. Better have a set of stirrups to stand in to relieve pressure, or a descender is a better option to help absorb shock and lower you to the ground. Check out the Tree stand Wingman.
Climbing harness plus a Ropeman 1.
I have my harness set up with a front belay device at my waist and use a long rope that will allow me to lower myself to the ground. Also have some pre-tied ropes in a drop bag hanging from the harness to use for foot loops to climb back up if I choose to do so.
