Default Notifying DNR of boat sale

The nice lady with dnr told me on the phone that there is a form, as the seller, that I need to fill out to notify them of my sale but I can't find it.

    Anyone help a brother out?

I didn't do this the last time I sold a boat and didn't realize it til I received a tax bill for boat/motor. I called, discussed, sent the bill of sale and it was all taken care of, until the same time next year. As it turns out, the guy I sold it to never registered it with them. PITA for sure but I suppose if I had completed this form it would/should have helped.
