    Yes and no. Some are flip/flop and some are not.

    Some heaters have larger elements in the bottom rather. For instance 4500w in the top and 5500w in the bottom, hence my statement concerning when the bottom element turn on.

    Never seen one. I don't understand what the benefit would be, especially since the top one comes on first anyway.

    True on initial fill and starting from cold. Normal operation, the bottom element takes priority because down pipe fills the cold water into the bottom. Hence why 90% of lower elements fail in comparison to upper.

    Yeah, not disputing with any of that. Not disputing anything at all. Just the both at one time (unless it's a commercial unit) and the higher wattage on just the bottom, unless it's just for quicker recovery.
